Dexametasona, dexmedetomidina, clonidina e morfina como adjuvantes na antestesia regional: uma revisão narrativa de segurança e eficácia

Resumo

Introdução: A curta duração dos anestésicos locais e a dor de rebote estimularam o uso de adjuvantes para prolongar a analgesia e melhorar o conforto pós-operatório. Objetivo: Analisar as evidências disponíveis sobre o uso de adjuvantes farmacológicos em bloqueios anestésicos regionais. Métodos: Revisão narrativa realizada nas bases PubMed/MEDLINE e Embase, abrangendo o período de 2000 a 2025, sem restrição de idioma. Após triagem e leitura dos textos completos, 21 estudos atenderam aos critérios de inclusão. As informações foram organizadas em quatro eixos: duração da analgesia, consumo de opioides, perfil de segurança e via de administração. Resultados: A dexametasona foi o adjuvante mais estudado, com aumento consistente da duração da analgesia e redução do consumo de opioides, sem diferença relevante entre as vias perineural e intravenosa. A dexmedetomidina mostrou eficácia semelhante, mas com maior incidência de bradicardia e sedação leve. A clonidina apresentou efeito moderado, limitado por hipotensão e sonolência, enquanto a morfina perineural prolongou a analgesia, associando-se a prurido e náuseas. Estudos recentes indicam efeito sinérgico entre dexametasona e dexmedetomidina, potencializando a duração do bloqueio e reduzindo o uso de opioides. Conclusão: Dexametasona e dexmedetomidina são os adjuvantes mais eficazes e seguros para prolongar a analgesia em bloqueios regionais. A associação entre ambos é promissora, mas requer validação em ensaios clínicos robustos. A padronização de doses e a avaliação da segurança neurológica a longo prazo permanecem prioridades para futuras pesquisas.
